Installment Agreement
The form the IRS sends to formalize an installment agreement (monthly payment plan) on back taxes.
Collection Information Statement for Wage Earners and Self-Employed
Detailed financial disclosure the IRS uses to decide what you can pay — required for most large installment agreements and Offers in Compromise.
Collection Information Statement (Short Form)
Shorter financial disclosure used by the IRS Automated Collection System for moderate balances.
Offer in Compromise
The form used to formally propose settling your IRS tax debt for less than the full amount owed.
Installment Agreement Request
The form used to formally request an IRS payment plan when you can't pay your tax in full.
Exercise of an Incentive Stock Option (ISO)
Reports the exercise of incentive stock options — drives Alternative Minimum Tax (AMT) calculation.
Amended U.S. Individual Income Tax Return
Used to correct errors on a previously filed Form 1040 — claim missed deductions, fix income, or respond to an IRS adjustment.
Power of Attorney and Declaration of Representative
Authorizes a licensed attorney, CPA, or enrolled agent to represent you before the IRS — speak, sign, and negotiate on your behalf.
Tax Information Authorization
Authorizes a third party (lender, accountant, attorney) to receive and inspect your IRS tax information — without representation rights.
Application for Automatic Extension of Time to File
Buys you six extra months to FILE your individual return — but does not extend the time to PAY what you owe.
Employee's Withholding Certificate
Tells your employer how much federal income tax to withhold from each paycheck — the single biggest lever for avoiding underpayment penalties.
Nonemployee Compensation
Reports payments of $600 or more to independent contractors — and is the IRS's primary tool for catching unreported 1099 income.
